Physical Therapist Job Description/Requirements:

• Evaluation/Treatment of Patients in their home. Driving is involved in this position.
• Assist in evaluating the level of function and in developing the plan of care.
• Evaluate the home environment and makes appropriate recommendations
• This is an independent contractor position and the pay is $85-115 per visit. Potential to make $150,000- 180,000 yearly.
• We are hiring in multiple locations across southern California for PT, OT, ST, PTA, COTA: such as Los Angeles, Van Nuys, Simi Valley, Northridge, Thousand Oaks, Santa Monica, Culver city, Inglewood, Southgate, Gardena, Torrance, Hawthorne, Long Beach, East LA, DownTown LA, South LA, Glendale, Eagle rock, Pasadena, Alhambra, Arcadia, Burbank, Diamond Bar, Hollywood, Beverly Hills, West Covina, arcadia.
• Riverside County, San Bernardino County: Rancho Cucamonga, Chino, Diamond Bar, Rialto, Riverside, Colton, Moreno Valley, Temecula, Corona.

Minimum Required Qualifications:

• Current CA Physical Therapy (PT) License
• Valid California Driver's License/ with transportation insurance
