Addison County Home Health & Hospice is looking for a full time physical therapist to visit patients in Addison County, VT. We serve patients in their homes and in community settings. We provide a plan of care developed by the therapist and approved by the physician. Using independent judgment based on common principles, our PT's provide early intervention services to individuals from birth to end of life. Qualifications include a current Vermont Physical Therapy license and a two years of relevant experience is preferred.
