Maternal Infant Health Program
SOCIAL WORKER
General Summary
Under the supervision of the Personal & Community Health Director, works with
Maternal Infant Health Program (MIHP) clients by assessing their social and emotional needs and identifying risk areas that could lead to an unhealthy pregnancy outcome or inability to effectively parent a child. Assists clients in obtaining community services.
Provides direct counseling services and psychosocial support to clients. Functions as part of a multi-disciplinary team to facilitate improved maternal and infant health outcomes.
Essential Functions
1. Participates as a member of the MIHP team, collaborates with other team members and is a team participant in developing a Plans of Care for clients.
2. Conducts MIHP visits in the client’s home or virtually.
3. Interviews client to obtain information concerning his or her economic condition and community and family support systems.
4. Assesses social and emotional factors related to client’s illness, need for care, response to treatment, adjustment to care or healthy prenatal/post partum progress.
5. Develops care plans based on input from the client, staff, and members of the client’s family, if applicable. Monitors implementation of service plans, conferring with nurses and other staff as necessary, and provides follow-up and assessment of services in relation to goals and objectives of the service plan.
6. Makes psychosocial assessments of clients needs including their strengths and weaknesses and progress or lack thereof towards a positive pregnancy outcome.
7. Functions as a case manager for designated clients, which includes the coordination between various agencies, medical provider and the client.
Provides comprehensive case management and coordinates services provided through a multidisciplinary approach collaborating extensively with multiple community agencies.
8. Networks and maintains good working relations with other community, agencies and advocates for clients by identifying needs to individuals and community and service agencies. Represents the agency at community functions and meetings.
9. Documents all client contact, maintains case records, and completes reports as required by department standards and the requirements of third party funding sources.
10. Other duties as assigned.

Employment Qualifications * Education:*__
Possession of a Bachelor’s Degree in Social Work (BSW).
Master’s Degree (MSW) preferred.
Experience: Prefer two years of professional social work experience providing services to families with intense needs. Prefer experience working with maternal and infant clients. Willing to train the right candidate.
Other Requirements: Possession of a valid State of Michigan driver’s license.
Michigan Social Work License or ability to get license within 1 year.
