PMC Regional Hospital is looking to add a Pharmacist to our team.

This position is part time, with a flexible schedule. Once established in the role, most of the work can be completed remotely, with visits to the hospital as requested to coordinate with staff and attend various committee meetings.

Job Summary:

Review of patient medication regimen, consultation regarding the disposal of medications, and any other service to be performed on-site by RN or Pharmacy Tech. Pharmacist shall, in accordance with the request of the Join Commission on Accreditation of Healthcare Organizations, serve as the Director of Pharmacy Services for the on-site licensed pharmacy.

Job Responsibilities:

• Perform routine audits of all medication storage areas and logs
• Perform spontaneous audits of controlled substance records
• Provide formulary recommendations and review
• Provide in-services pertaining to adverse drug reactions, IV medications, antibiotic utilization, look-alike/sound-alike medications, high alert medications, and do-not-use abbreviations
• Review medication errors
• Participate in performance improvement activities
• Provide drug recall/shortage information
• Serve as drug information resource
• Provide policy and procedure manual review
• Participate in pharmacy and therapeutic committee/medical executive committee
• Communicate and coordinate medication use process for patients
• Availability for participation is QA, Pharmaceutical Service, IC, UR committees and antimicrobial stewardship
• Drug regimen and medical records review
• Supervision of the records of receipt and disposition of all controlled drugs

Qualifications and Requirements:

• BLS certified within 30 days of hire
• PharmD or equivalent
• Licensed pharmacist in Indiana
• Knowledge of and ability to support populations served, infant through older adult

Pay: $54.00 - $58.00 per hour
