L
iving Independence For the Elderly (LIFE) is a managed care program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in the community through supportive services. The LIFE centers provide a single hub for primary care, pharmacy, nursing, restorative therapy, adult day services, care coordination and social work. The center staff coordinates additional supportive services such as home health care, transportation, home accessibility modification and home delivered meals. LIFE coordinates and provides all needed preventative, primary, acute and long-term care services. All of the team members work together to allow participants remain in their own home.

The primary responsibility of the incumbent is to provide counseling services for LIFE participants including one on one and group counseling. The incumbent is also expected to provide counseling updates to IDT, facilitate psychiatric appointments for participants to outside providers, follow participant’s mental health needs in other settings, and to be a liaison for the participant’s mental health needs in the LIFE program and in other settings. At times, the incumbent will be required to support the social work department in traditional social work roles as determined by the social work manager.
Qualifications:
  • Master’s Degree (MSW) in Social Work from an accredited school of social work.
  • Minimum of one (1) years’ experience working with a frail or elderly population; Thorough working knowledge of current community health practice for the frail elderly from direct service experience; Working knowledge of the interdisciplinary model of care management; Experience working with cognitively impaired seniors; Must be able to relate well with seniors and their families to deal with sensitive issues and facilitate problem solving; Must be flexible.
  • Working knowledge of computers, Microsoft Word and Excel required.
