Position Summary
Responsible for coordinating care for participants in conjunction with the Interdisciplinary Team, ensuring that participant’s emotional and psychosocial needs are met through the duration of their enrollment. Continually assess participant’s needs and communicate and advocate those needs to IDT. The MSW must be capable of problem solving both independently and in a team setting utilizing masters level interventions.
Required Qualifications
- Must possess a Masters degree in social work (MSW) from a program accredited by the Council on Social Work Education (CSWE)
- Must have, as a minimum, two years social work experience working directly with individuals
- Must have reliable transportation and valid drivers license
- Minimum of one year of experience working with the frail elderly, or long term care population
