We are searching for a highly organized and energetic individual to join our team as an Outpatient Registered Nurse. Reporting to the Clinical Supervisor, the Registered Nurse works closely with a treatment team of other mental health professionals including psychiatrists, psychologists, and clinicians in monitoring the physical and mental health status of the clients receiving Outpatient and/or Psychiatric services. This is a full-time position with hours of work aligning with the clinic's needs and operations.

What Do I Need?
Education and Experience
Licensed RN in State of Pennsylvania
AND
Two or more years of experience in a healthcare environment, preferably Mental Health.

Other Requirements

• You will exhibit average competency with Electronic Health Records (EHR) system.
• You will possess a valid driver’s license and have the ability to travel as needed.

• You will maintain required state and federal clearances and/or credentials.

What Will I Do?

• You will create global awareness of stakeholders throughout the behavioral health system on Physical Health and Behavioral Health (PH/BH) issues.
• You will monitor the physical and mental health status of clinical clients and identifies individuals who have extensive, co-morbid PH/BH conditions.
• You will coordinate nursing care and services with the multiple disciplines of the treatment team.
• When necessary, you will accompany BHH individuals to medical appointments to ensure optimal care and offers follow-up consultations and monitoring aligned with the physical health provider’s recommendations.
• You will analyze, manage and report on individual as well as group PH/BH care utilization data from PH/BH Managed Care Organizations to include designing and conducting health surveys for co-morbid cases.
• You will maintain medical charts and records for BHH clients with high standards of confidentiality as it relates to HIPAA regulations and Agency expectations.
• You will request and assess medical reports from outside agencies.
• You will provide BHH wellness coaching and training for Clinical Therapists, Psychiatrists and other behavioral health staff as needed.
• You will monitor all BHH contractual obligations and develop plans to achieve outcome requirements.
• You will recognize overuse of PH/BH services through Clinical Therapists and offer assistance in developing an individualized wellness plan.
• You will generate and implement programs around compliance to medication and medical appointments.

This is a full-time position with hours of work aligning with the clinic's needs and operations.
