POSITION: Electronic Health Liaison - Registered Nurse

JOB SUMMARY:

This role is a Registered Nurse that directly supports primary care providers in Electronic Health Record (e.g. MyChart) message management. The RN will work under the supervision of the Clinic Nursing Leadership team with close collaboration with the clinic medical director and primary care department physician leadership to optimize EHR message management through the electronic health record (EHR) system to support the ever-growing population of patients who rely on EHR messaging to access health care. The RN is the first line of contact to patients in response to EHR messages. The role is to respond to all EHR messages by offering clinical guidance, triage to higher level of care, and patient education prior to heightening to provider level, if indicated. This role will also be able to offer development in optimizing patient communication through electronic platforms through patient education on best use of EHR messaging, phone call, virtual visits, and face-to-face visits. This position is a remote position after 90 days of successful orientation. Some on-site work may be required upon request of employer for additional training, provider collaboration, and meetings.

QUALIFICATIONS:

    Currently registered with the Minnesota Board of Nursing.
  • Bachelor's degree preferred.
  • Current BLS certification or obtain within 2 months.
  • Demonstrates clinical competence in providing direct patient care, patient and family education, and assisting the provider in the coordination of patient care.
  • Demonstrated abilities to manage multiple priorities and organize workload; includes critical thinking, delegation skills, time management, and respectful communication.
  • Three years of experience working in the ambulatory interface of Epic EHR, triaging of patients by using clinical judgement within the RN scope, and care coordination in the health care environments preferred.
  • Motivational Interviewing experience preferred.
  • Skill in coordinating care needs.
  • Must meet additional requirements to be eligible to work remotely after 90 days of onboarding.
