Job Functions:

• Administers direct patient care in accordance with the written and signed prescription of the referring physician.
• Designs a treatment program to achieve established goals. Provides goal-directed activities that require the skills of an Occupational Therapist consistent with the plan of care.
• Submits required documentation in compliance with applicable state and federal home health related rules and regulations, agency requirements and/or third party payer requirements.
• The patient’s plan of care shall be updated as required by regulatory bodies, by the occupational therapist when the content of the plan has changed, at least every sixty days or as patient’s condition warrants an update.
• Address upper extremity function, safety needs, ADL skills, functional mobility, and energy conservation for maximum functional activity level in the appropriate patient assignment.
• Each visit/patient encounter requires that documentation be completed and included in the patient’s clinical record.
• Conferences with family members/other care providers involved in or supervising care are to be documented.

Qualifications:

• Graduate of an approved college with a B.S. or higher degree in Occupational Therapy.
• Licensed and registered with the State of New York.
• Certified with NBCOT (National Board for Certification in Occupational Therapy) desirable.
• Experience with the geriatric population preferred.
• One year experience with home care preferred.
• Valid driver’s license and/or access to transportation.
