The Role:

Hospice Care is a service offered to our patients that focuses on quality of life and comfort when living with a serious illness.

We’re looking for MSWs that want to use their expertise and experience as part of a team to provide the best care possible to those experiencing serious illness.

By providing expert, personalized care to hospice patients where they live you will bring dignity and compassion to those coping with symptoms of a serious illness.

Requirements:

  • Master’s Degree in Social Work (MSW) required
  • Minimum of one-year experience in Hospice preferred
  • Demonstrated experience working with multigenerational family systems in a clinical capacity
  • Demonstrated experience working with life-threatening conditions, chronic disease, hospice and / or end-of-life care, and related situations
  • Excellent verbal and written skills
  • Must have reliable transportation, valid driver’s license, and proof of auto insurance in order to travel to and from patients’ homes
  • Must be fully vaccinated against COVID-19
  • Tuberculosis screening after hire required

Responsibilities:

  • · Participate in planning and coordinating patient care in collaboration with patient/family and other members of the health care team to maximize patient outcomes and use of resources.
  • · Assesses patient's emotional, social and spiritual status initially and on an ongoing basis.
  • · Utilizes psychosocial assessment to develop a care plan with measurable goals and individualized interventions.
  • · Utilize a variety of techniques to identify and assess the psychological, emotional and social factors affecting the patient and family and communicates patient and family needs to the interdisciplinary team.
  • · Provides counseling to patients and families experiencing emotional, social or economic stress related to terminal illness.
  • · Develop plans of care based on the initial assessment, advance care planning, educational components as needed.
  • · Engage with patient and family to facilitate trust, rapport, and communication and provide supportive counseling, crisis intervention, and/or appropriate therapeutic supports as needed to the patient, family group or individual family members.
  • · Act as liaison to promote discussion between the patient, family, and health care team, in order to facilitate understanding of the patient’s medical condition and treatment options, and to establish clear, patient-centered goals of care and care plans.
  • · Provide instruction/education/counseling/guidance consistent with learning needs, abilities and readiness of the patient and family in preparation for end-of-life care, death and healthy management of grief and loss.
  • · Refer patients and families to appropriate supports and resources, including Agency services such as spiritual counseling, aides, or Massage or Music Therapy, and community-based services including, mental health resources, bereavement counseling, family services, and related legal services.
  • · Coordinate the utilization of community resources including financial resources for needed caregiving, supplies, equipment and medications.
  • · Advocate for the patient and family to ensure respect for patient autonomy and to maximize the safety of the patient’s care plan.
  • · Ensure the patient/family have choices in their care and have a right to maintain as much control as possible over their lives.
  • · Acts in a primary role of assessing bereavement risk and providing information for bereavement follow-up.
  • · Participate as a member of the larger Hospice Care Team, communicating clearly and effectively with other team members to provide quality patient and family care, meet individual patient and family members care needs, and achieve appropriate hospice care plans.
  • · Consistently and accurately documents patient condition and interventions in a systematic, concise, legible, and timely manner according to established agency guidelines.
  • · Convey a supportive attitude and establishes and maintains open lines of communications that are clear, concise, and informative.
  • · Demonstrate sound knowledge and skill base in support of hospice philosophies.
