Job Title: Nurse - RN or LPN (Day Shift) $4,000 SIGN ON BONUS!

Schedule: Part-Time or Full Time Days and Weekends

Pay: $24-$28/hour (LPN) | $35-$40/hour (RN)

Sunflower communities has an opportunity for hardworking and caring nurses to join our team! Successful applicants will have prior experience in caring for the older adult population.

Nurse - RN / LPN Overview:

• Oversee the coordination and plan of residents’ care, ensuring care practices have each resident’s best interests in mind
• Complete clinical assessments of residents’ health status and document appropriately
• Monitor the status of chronic illnesses and manage the administration of medications
• Coordinate with external health care providers and internal team members
• Interact with residents’ families and loved ones, reporting changes in their health or care plan

Nurse – RN / LPN Qualifications:

• Licensed as a Registered Nurse / RN or Licensed Practical Nurse / LPN in Minnesota
• Experience with Electronic Medical Records (EMR) software
• Prior nursing experience in a SNF, AL, TCU, or Home Health setting is ideal
• Demonstrated ability to work autonomously
• Excellent at planning, organizing, and communicating
