Palliative Clinical Social Worker (LCSW required)

We are looking for an experienced, compassionate Palliative Clinical Social Worker to join our team on a schedule of 32 hours per week. We are flexible how you work those hours. What schedule works best for you? No nights, no weekends and no on-call in this role.

This role is spent in the community, meeting with patients and their families in a variety of settings including; homes, facilities, hospitals and more.

JOB SUMMARY:

The Palliative Clinical Social Worker is a primary member of the Palliative Care Team and is responsible for providing psychosocial education to patients and families.

The PCSW assists with the coordination of care and is responsible for facilitating communication around advanced directive planning and anticipatory grief.

The PCSW assesses and navigates complex family dynamics; assesses for psychological stressors that amplify symptoms and provides resource counseling for caregiver resources, alternative levels of care, financial and legal needs.

EDEUCATION & QUALIFICATIONS:

  • Graduate of an accredited master’s degree in a Social Work program
  • LCSW required
  • Minimum of two years of hospice or palliative experience preferred
  • ACHP-SW Certified Preferred: If not certified, goal to become certified within 1 year of hire date
  • Must have a National Provider Identifier or be eligible to apply for one
  • Must successfully pass agency required background checks and drug screening
  • Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page”0 indicating you have $100k/$300k Bodily Injury/$100k Property Damage coverage) and have reliable transportation
  • Requires local travel to multiple work locations
  • Must comply with agency health screening and immunization requirements

ESSENTIAL DUTIES:

  • The PCSW will provide telephonic and in person clinical assessment, advice, counseling, and referral facilitation
  • Identifies the patients' and family’s psychosocial, environmental, safety and developmental needs and participates in individualizing the patients' plan of care based on those needs
  • Incorporates patient/family preferences and goals within the plan of care
  • Supports and respects the patient’s and family’s unique spiritual and cultural beliefs
  • Documents assessments, interventions, outcomes, follow-up, and communication with IDT for patient visits; completes documentation in an accurate and complete manner in compliance with regulatory standards and within agency standards
  • Facilitates family meetings
  • Identifies community resources for the patient and families
  • Supervises students and interns as requested
  • Knowledgeable of Medicare and Medicaid hospice and palliative care benefits and regulatory compliance requirements
