  • Must have a current practice license, driver's license, CPR/ACLS certifications
  • Works independently in the patient's home to conduct evaluations and follow-ups as needed
  • Carry out appropriate exercises, and treatment plans
  • Documents appropriately and timely using EHR system
  • Ability to supervise Physical Therapy Assistants
  • Ability to document and communicate well with the Rehabilitation team
  • Reports to Director of Patient Care Services
