Position: Certified Medical Assistant/Patient Care Coordinator

Supervisor: Director of Nursing

Division: Tribal Academic & Human Services Administrator

Department: Kickapoo Community Health Center

Responsibilities

Responsible for the effective coordination and management of all patient referrals (internal and external) to ensure continuity of care. Refers patients to specialists and/or diagnostic testing in accordance to referral protocols and as directed by the provider. Works with insurance companies and obtains authorizations for patient referrals, diagnostic testing and procedures. Responsible for scheduling, tracking, referral case management and providing reports in accordance to Referral Management Policies. Represents the KCHC professionally and positively exercising utmost discretion and tact in customer interactions. Performs clinical and administrative duties as a certified medical assistant when directed by the Supervisor.

Essential Duties and responsibilities include the following:

· Process patient referrals to specialty providers by preparing health records for referral patient visits and/or providing appropriate clinical information to specialist

· Ensure complete and accurate patient demographics and current insurance information.

· Assemble information concerning patient’s clinical background and referral needs and provide appropriate clinical information to specialist or service by phone, fax or email while maintaining HIPAA compliance.

· Obtain authorizations for patient referrals to specialists, diagnostic testing and procedures. Advise provider if authorization request is denied.

· Work with PRC Department for referral of tribal patients without insurance and for transportation as necessary.

· Review details and expectations about the referral with patients.

· Assist patients in problem solving potential issues related to the health care system, financial or social barriers (i.e. request for interpreter, transportation services, prescription services).

· Remind patients of scheduled appointments via mail or phone.

· Maintain ongoing tracking and appropriate documentation on referrals by recording all referrals into a referral log as well as transitioning all communications into EHR documentation.

· Ensure that referrals are addressed in a timely manner, prioritizing urgent items appropriately.

· Identify and utilize cultural, language and community resources; establish and maintain relationships with identified service providers.

· Act as resource for patients or providers who have questions or problems relating to referrals.

· Ensure that patient’s electronic health record is up to date with information on specialist consult, hospitalizations and/or procedures related to their health.

· Conduct pre-visit planning by reviewing and updating patient charts prior to appointment with recent test results and correspondence and determines needed services per practice protocol.

· Confer with provider regarding any incomplete patient tests/consults or other incomplete orders prior to patient visit.

· Prepare patient for examination and treatment by escorting them from the waiting area to the exam room and providing them with a patient gown.

· Interview patients to obtain medical information and measures their vital signs, height and weight.

· Prepare and administer medications, including injections and immunizations, as directed by the provider.

· Assist laboratory in collecting blood or other laboratory specimens, labels the specimens and conducts waived testing as necessary.

· Explain treatment procedures, medications, diets and provider’s instructions to patients.

· Support patient care delivery by authorizing drug refills as directed; telephoning prescriptions to the pharmacies; contacting medical facilities or departments to schedule patients for admission, testing or procedures.

· Assist with ECGs, OB/GYN exams and office procedures as needed; and provide other patient care services as directed by the provider.

· Clean and sterilize instruments and disposes of contaminated supplies.

· Prepare and maintain clinical supplies by inventorying stock; placing orders; verifying receipt.

· Keep equipment operating by following operating instructions, troubleshooting breakdown or calling for repairs

· Prepare, clean and maintain exam and treatment rooms.

· Assist the provider in the transfer of patient to the hospital by activating the emergency response number (911); ensure family is notified of the disposition of the patient

· Discharge patients as directed by the provider ensuring that the patient receives and is instructed on the Plan of Care and provided any additional information.

· Assist the clinical team in responding to medical emergencies in the clinic by bringing emergency equipment to the site of the emergency, providing CPR as necessary and/or contacting family or 911 as directed by the provider or nurse.

· Will respond to emergency/disaster situations in accordance to the Emergency Preparedness Plan and assist the provider and/or in providing immediate medical assistance such as first aid as directed.

· Assist nurse with home visits as assigned by preparing supplies, equipment and PPE, forms as indicated .

· Work with patients who have communicable diseases (i.e. COVID-19, TB, HIV) ensuring that infection control standards and CDC guidelines are followed.

· Assist the provider in caring for patients in the COVID-19 Unit with screening, testing, counseling, teaching and promoting infection control standards in accordance to CDC guidelines. May assist RN/LVN with home-visits/community support to meet the unique needs of the Tribe.

· Participate in outreach activities (i.e. health fairs, community events) to promote healthy lifestyle changes, provide basic health screening and/or for training and education.

· Employee must maintain confidentiality, security and integrity of patient data and information according to privacy regulations, including HIPAA. Employee assures age and culturally appropriate, holistic patient care based on mission, standards and policies; adheres to standards of relevant regulatory agencies. Employee must consistently be courteous to patient, staff and visitors. Employee participates in clinic performance improvement activities to measure, assess and improve the quality of patient care.

Education/experience requirements:

Minimum Qualifications: Graduate of a formal medical assistant training program.

Two-years’ experience in a clinic or physician office setting.

Preferred Qualifications: Previous work related experience is preferred.
