UW MEDICAL CENTER - MONTLAKE SOCIAL WORK has an outstanding opportunity for a PART-TIME SOCIAL WORKER (50% FTE, Night Shifts) within the Emergency Department. This position reports to the Emergency Room Social Work Manager. This social work professional will demonstrate the knowledge and skills to provide care and/or services appropriate to the age of the patient in the service area.

RESPONSIBILITIES:
  • Consult with medical and nursing staff in order to screen patients for service needs, provide information regarding social work activity and to coordinate care as a part of an interdisciplinary team.

  • Conduct independent psychosocial assessments and psychiatric evaluations, facilitating psychiatric admissions at UWMC, other hospitals or via involuntary treatment services, as appropriate.

  • Provide consultation with other UWMC Social Work Department team members.

  • Provide telephone consultation and crisis intervention for persons calling in from community.

  • Provide thorough documentation of clinical activity via medical record for continuity of care and legal issues.

  • Psychosocial and physical needs assessment of medically ill EMS patients.

  • Arrange visiting nurse or chore services as needed.

  • Alert inpatient social worker of patients admitted needing discharge planning.

  • Substance abuse assessments and treatment referrals.

  • Domestic violence assessments, referrals to safe houses and community resources.

  • Liaison between medical staff and family of EMS patients in medical crisis – information, support, grief work as needed.

  • Function as patient advocate in EMS; provide information, support basic comfort measures; help clarify patient needs to medical staff.

  • Participate as an active member of a Clinical Social Work Team contributing to the successful functioning of the Social Work Team and Department Submit statistics on daily activity.

  • REQUIREMENTS:

    A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND one year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

    LEGAL REQUIREMENT:
    Within 90 days of hire must hold one of the following credentials issued by the state of Washington:
  • Agency Affiliated Counselor

  • Licensed Social Work Associate -Advanced

  • Licensed Social Work Associate - Independent Clinical

  • Licensed Advance Social Worker

  • Licensed Independent Clinical Social Worker
