POSITION SUMMARY

Help to plan, organize, develop and direct the overall operation of the Social Services Department in accordance with Federal, State, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

• Help to plan, develop, organize, implement, evaluate, and direct the social service programs of this center.
• Develop and implement policies and procedures for the identification of medically related social and emotional needs of the resident.
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Interview resident/families as necessary and in a private setting.
• Involve the resident/family in planning social service programs when possible.
• Refer resident/family to appropriate social service agencies when the center does not provide the services or needs of the resident.
• Coordinate social service activities with other departments as necessary.
• Assist the CQI in developing and implementing appropriate plans of action to correct identified deficiencies.
• Interview residents, or family members, as necessary, to obtain social history.
• Assure that social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
• Coordinates, Implement and schedule the care planning process and oversees the ICT.
• Serve on, participate in, and attend various committees of the center as required, and as appointed by the Administrator.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactory. The requirements listed below are representative of the knowledge, skills and abilities required.

Educational Requirements and Experience:

• Must possess, as a minimum, a Bachelor’s Degree in Social Worker or a related hospital field.
• Must be licensed to practice Social Work in the State of Texas.
• Minimum 2 years of experience as a Social Worker.
• Knowledge of Skilled Nursing and Long-term Care.
