This position coordinates medically related social services to nursing home residents.
Duties include:

• Care planning
• Discharge planning
• Advocating for residents’ rights
• Providing emotional support to residents and their family members.

Qualifications:

• Must possess a bachelor’s degree in a human service field.
• Must be Certified Social Worker in New Jersey
• Experience working in a nursing home setting preferred.

Job Type: Part-time 24 Hours per Week
