RESPONSIBLE FOR –

• Responsible for nursing assignments of health needs.
• Will contribute to the plan of care for individuals and families enrolled in the LIFE Pittsburgh Program.
• Working with a multi-disciplinary team to create and implement nursing care plans and family nursing needs.

SHIFT –

• Primary working hours are Monday thru Friday 8:00 AM – 4:30 PM with rotating on-call shifts. Full and Part Time position available.

LOCATION/TRAVEL REQUIRED –

• Positions based out of an Adult Day Health Center (located in North Shore, Allegheny Center, McKees Rocks and Green Tree)
• Travel to, and working in Participant residences, hospitals, nursing home facilities or other settings in which the Participant may need support.
• All business mileage is reimbursed at the Federal rate.

LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community-based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric physicians and nurse practitioners, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in-home program according to an individual Participant's needs.

REQUIREMENTS –

• Minimum of 1 year experience providing direct nursing care and oversight of Geriatric patients
• Valid Pennsylvania RN License
• PA Driver’s License with access to a vehicle
• Experience working with and relating to the geriatric populations and their family members is preferred.
