PHYSICIAN ASSISTANT

We are seeking a Full Time Physician Assistant to work in our skilled nursing facility located in Poughkeepsie, NY. This autonomous position will be providing care to a dedicated patient panel in the facility during regular office hours. Monday - Friday. No weekends/holidays/call.

RESPONSIBILITIES

• Evaluates, diagnoses, treats, and manages acute, critical and chronic patients illness and diseases.
• Obtains medical histories, conducts physical examinations and assesses and determines differential diagnosis. Prescribes medication, therapy and treatments accordingly.
• Interprets and evaluates patient data for the purpose of determining management and treatment of patients. Consults with physician as appropriate. Orders, performs, and interprets diagnostic studies in the field of clinical specialty.
• Adjusts patient care plan by reviewing and monitoring treatments and therapy plans.
• Provides care coordination, which includes managing a variety of ancillary services, pre-operative plans, provider referrals and medical devices such as insulin pumps, defibrillators, and pacemakers.
• Prescribes, recommends and regulates pharmacologic and non-pharmacologic therapies across multiple care settings, including controlled medications that are warranted by patient's diagnosis or changing medical condition.
• Prepares, dictates and signs patient care plans and summaries of patient care. Maintains records according to established standards and guidelines.

We provide care 24-hours a day by combining bedside clinicians with on call service. Our bedside providers work at a single location and care for a small, defined patient panel. Our on call team covers our facilities after hours and on weekends. They have the ability to utilize telemedicine when appropriate. They have access to medical records. They collaborate with nursing, establish care plans in acute situations, communicate with families and coordinate cares when referrals offsight are required. Our bedside providers and call team are a cohesive group. Some of our call providers provide onsite medicine when we transition into new buildings. This allows us to introduce our care models to both facility staff and providers new to our team. T4 is provider owned and operated. The providers on our leadership team continue to practice bedside medicine and share in call coverage. We provide proactive medicine. We strive to be evidence driven.
