Summary
This position provides physical therapy services to participants of the AllCare PACE program.
Essential Duties
1. Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participation in, interdisciplinary team meetings; collaborates on care planning and service allocation decisions.
2. Completes participant evaluations as needed and develops a plan of care. Evaluations include but are not limited to the following: gait assessment, balance, bed mobility, functional status, physical status, home safety and environmental assessments, restraint assessments, and wheelchair needs.
3. Performs initial and annual screenings/assessments as assigned and makes appropriate recommendations for services.

Education & Experience
Bachelor's degree (BA) from a four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
Certificates, Licenses and/or Registrations
Current BLS certification.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills
Ability to apply advanced mathematical concepts such as exponents, logarithms, quadratic equations, and permutations. Ability to apply mathematical operations to such tasks as frequency distribution, determination of test reliability and validity, analysis of variance, correlation techniques, sampling theory, and factor analysis.
Computer Skills
Job requires specialized computer skills. Must be adept at using various applications including database, spreadsheet, report writing, project management, graphics, word processing, presentation creation/editing, communicate by e-mail and use scheduling software.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Physical Demands & Work Environment
The employee must occasionally lift and/or move up to 10 pounds.
