Physical Therapist

Requirements:

• Organizational skills
• Positive Attitude
• Technology Savvy "Kinnser"
• Current license valid in the State of Texas
• Minimum one year full-time experience
• Valid driver’s license
• Current auto insurance
• Current CPR
• Current TB test (can be provided by the company)
• Reliable Transportation
• Must be able to speak, read, write and understand fluent English

Duties will include:

• Perform Initial Patient Evaluations
• Develop, review and update home therapy care plans in coordination with the patient's physician
• Perform 30-day re-evaluations, post-hospital evaluations and discharge evaluations
• Administer, instruct, motivate and assist patients in therapeutic exercises
• Care coordination with the interdisciplinary team, physicians, patients and families
• Conduct discharge planning and complete discharge summaries
• Patient teaching
• Record progress notes, report changes in patient's condition and response to therapy
