Job Description :

Qualifications include:

  • Graduate of an accredited nurse practitioner program and completion of Masters of Science Degree in Nursing – OR - Graduate of AMA accredited physician assistant program.
  • Current Wisconsin license as a Registered Nurse and recognition as an Advanced Practice Nurse by the Wisconsin State Boards of Nursing – OR Current license as a Physician Assistant by the Wisconsin Boards of Medical Practice
  • Current BLS and ACLS Certifications
  • Must possess active DEA Number.

Schedule:

2 days per week in Baldwin, WI.

Duties include:

This role works in collaboration with physicians and other medical staff to provide services to patients and their families. They are engaged in independent decision-making about health care needs and provides health care services to individuals and their families. Performs a comprehensive history, including chart review and information gathered from the family and performs physical and psychological health assessments. This role will also assist in the development of a cancer survivorship program and serve as the primary clinical provider in the survivorship clinic.

  • Assess health status for specific patient populations using advanced practice knowledge and skills
    • Performs clinical examinations and consultations with cancer patients and cancer survivors seeking care. Include generation of cancer treatment history for cancer survivors.
    • Obtains relevant health and medical history
    • Performs a physical examination based upon age and medical history
    • Orders and interprets necessary procedures and diagnostic tests
    • Identifies physiological and psychological conditions
    • Identifies patients’ symptoms and goals and serves as an advocate with the health care providers
    • Synthesizes information from the history, physical and diagnostic tests to develop, document and implement a Treatment Plan or Survivorship Plan
      • Establishes a practice agreement with the collaborating physician
      • Collaborates with the patient’s primary physician in a consultative role
      • Collaborates with all of the patient’s physicians and other health care team members in developing realistic treatment/survivorship goals
      • Develops a problem list based upon physiological and psychological disorders
      • Develops and implements a treatment/survivorship plan
      • Communicates effectively with patients and families for understanding of diagnosis, prognosis and plan
      • Determines the effectiveness of the treatment/survivorship plan and documents patient care outcomes
      • Revises and modifies the treatment/survivorship plan to meet the patient’s goals
      • Knowledge of pharmacology principles. Prescribes pharmacological (including controlled substances) and non-pharmacological (including controlled substances) and non-pharmacological agents
    • Develops and Promotes Survivorship Program to patients, families, community, and other health care professionals
      • Assists with writing, identifying, and implementing program eligibility guidelines, evident based protocols, standardized procedures and patient questionnaires
      • Develops relationships with other specialties that can assist with the management of cancer survivor problems
      • Provides education to patients, families or caregivers as needed
      • Provides continuing education to community and other professional groups as requested
    • Collaborates and coordinates with other health professionals
      • Makes and accepts referrals to//from, consults with, cooperates with and refers to all other types of healthcare providers, including primary and specialty care physicians, nurse practitioners and other allied personnel
      • Discriminates between situation requiring independent management, collaborative management with physician and further referral
      • Collaborates with physician when unusual incidents or changes in patient’s condition occur
      • Coordinates care involving other healthcare providers
      • Functions as mentor/resource and educator for other healthcare team and staff
