Anchor Home Health Care has Full or Part-Time opportunities for an Occupational Therapists to service Lake or Porter County, IN.

Anchor has NO minimum number of required work hours per day or work days per week for part-time/PRN employees.

POSITION SUMMARY

The Field Occupational Therapist is a qualified professional who directs, supervises, evaluates, and provides occupational therapy services to patients in the home as prescribed by the physician. He/she is responsible for: (1) providing skilled therapy care to acutely ill, and/or chronically ill patients in the home: (2) recording clinical and progress notes: and (3) coordinating other patient care activities/services to provide the highest quality of patient care.

POSITION QUALIFICATIONS

Licensed as an Occupational Therapist in the state of Indiana.

Is a graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or Is eligible for the National Registration Examination of the American Occupational Therapy Association; or has 2 years of appropriate experience as an occupational therapist, and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service, except that such determinations of proficiency do not apply with respect to persons initially licensed by a State or seeking initial qualification as an occupational therapist after December 31, 1977.

Speaks, writes, reads and comprehends English.

A minimum of one year of acute care experience required; previous home care experience preferred.

Must possess strong patient assessment, patient management, and documentation skills.

Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.

Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team.

Absolute integrity and the use of good judgment to handle the professional and confidential nature of assignments.
