Description
The Department of Family, Population and Preventive Medicine is seeking a Nurse Practitioner or Physician Assistant to provide direct care to patients in the Division of Family and Community Medicine at Stony Brook, in collaboration with an Attending Physician.
Responsibilities:
- Participate as part of a multidisciplinary team to provide continuity of care and follow-up.
- Provide direct care, counseling, and teaching to patients and families according to established collaborative protocols and standards of care.
- Evaluate outcomes of patient care and make necessary revisions in care plan.
- Through patient education and counseling, incorporate the concepts of health maintenance, prevention, and promotion into daily practice.
- Identify own learning needs and assume responsibility for maintaining current knowledge and competency in clinical practice area.
- Participate in the quality assessment/improvement program for the service.
Qualifications
REQUIRED QUALIFICATIONS
Nurse Practitioner:
Current NYS RN Licensure, NYS certification as an FNP; completion of an accredited Nurse Practitioner course of training. Masters of Science degree, licensed in the State of New York and board certified by a Nurse Practitioner Certifying body (AANC, AANP, AACN, NCC, PNCB). Excellent communication skills (both oral & written), interpersonal and organizational skills are required. Basic Computer skills required.
Physician Assistant:
Bachelors Degree from an accredited Physician assistant (PA) school, current license and certificate to practice as a Physician Assistant (PA-C). Excellent communication skills (both oral & written), interpersonal and organizational skills are required. Basic Computer skills required.
