Nurse Practitioner -Hospice
OPPORTUNITY FOR GROWTH
Under the direction of the Medical Director/Hospice Physician and the Executive Director, and in accordance with Athena Hospice of Rhode Island policies and procedures, is responsible and accountable for performance of a face to face assessment for assigned patients in consultation with the hospice Medical Director/Physician. In collaboration with the IDG (interdisciplinary group), patient and family, provides consultation, assessments and evaluation of hospice eligibility.
ESSENTIAL FUNCTIONS:
1. Documents medical history, pertinent patient information, hospice eligibility, disease progress, functional and physical decline, medications, and/or diagnostic studies as required for the Face to Face Encounter patient visit. (per agency documentation standards).
2. Communicates findings and changes to RN case manager and/or Medical Director/Hospice Physician.
3. Educates/counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients.
4. Reviews, follows, and/or updates written plan of care in coordination with an interdisciplinary Team (IDT).
5. Assures that patient receives appropriate measures to control symptoms, through collaboration with IDT members.
6. Document care provided in the patient’s clinical record providing evidence of the progression of the end-stage disease process.
7. Attends Physicians meetings and other meetings as directed by Medical Director/Hospice Physician or Executive Director.
8. Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth.
9. Maintain current knowledge of trends in hospice care and pain/symptom management
10. Work in a collaborative team approach with the IDG
11. Participate in the QA/PI process as required
12. Provide consultation & education to colleagues and attending physicians related to admission criteria for hospice and palliative care
13. Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
14. Participate in resolution of interpersonal conflict and issues of clinical and ethical concern
15. Maintain and enhance professional growth and development through attendance at seminars, conferences, and continuing education offerings.
16. Act as a professional role model by demonstrating behavior consistent with the philosophy, mission and purpose of the hospice.
17. Adhere to all Confidentiality policies and procedures
18. Perform other related duties or special projects as directed by the Executive Director and/or Medical Director/Hospice Physician
19. Performs all services and duties in strict accordance with all laws, rules, regulations, ordinances, and judicial and administrative interpretations thereof, of the United States, and the State of Rhode Island.
