The Hospice/Palliative Care Nurse Practitioner contributes to the palliative care program and provides quality end of life care to patients and families. Clinical responsibilities include education and consultation for complex pain and symptom management, end-of-life decisions and advance care planning. Face to face hospice visits and general support to hospice program.
