Job Summary

This individual implements targeted case management based on the individual client's needs and resources, and on clinic and community resources. The social worker also serves as a resource person to the medical center staff. Coordinates professionals involved in patient care to insure interdisciplinary collaboration and continuity of services.

Duties & Responsibilities

  • Interviews patient and family members to obtain information about home environment, family relationships, and health history. Performs accurate social work assessment.
  • Evaluates data and establishes a case management plan that provides maximum benefit to the patient.
  • Involves patient in identifying and understanding problems.
  • Explores available resources and services. Consults with other caregivers.
  • Educates and supports patient regarding options and services available.
  • Provides for a multi-disciplinary approach to treatment plan, coordinating with other medical staff.
  • Coordinates care of patients with referring physician or agencies.
  • Maintains documentation on all case files including patient information, goals, progress, and outcomes.
  • Prepares periodic reports of work accomplished and compiles specific statistical data to support program goals.
  • Attends and participates in conferences to discuss patient's care and other staff meetings. Participates in committees as requested.
  • Maintains current knowledge of community resources and makes referrals which adequately and appropriately utilize these resources. Provides updated information to other clinic staff regarding community resources.
  • Works with collateral personnel as needed in the planning and delivery of services and case management.
  • Provides assessment (by telephone and in person) of crisis situations.
  • Offers inservice education programs and functions as a resource to clinic staff and the community as needed.
  • Participates in development and selection of patient education materials and services.
  • Represents the clinic to the community at large in a professional and helpful manner.
  • Maintains pertinent patient information in the medical record, and completes documentation to promote timely patient billing,
  • Carries out all duties with the highest professional standards, including maintenance of patient confidentiality.
  • Fulfills additional responsibilities as requested.

Knowledge, Skills, & Abilities

Knowledge of the principles and practices of social work and counseling techniques. Knowledge of community resources and referral agencies. Knowledge of group dynamics. Skill in interviewing and communicating to elicit information and cooperation from patients. Skill in establishing and maintaining effective working relationships with patients, staff, and the public. Ability to interpret and communicate social work agency rules, regulations and procedures. Ability to write reports and document patients' progress.

Qualifications

Bachelor's degree in Social Work.

At least two (2) years social work experience preferred.

Prior experience working with HIV population highly desirable.

Typical Physical Demands

Requires sitting for long periods of time. Occasional bending, stretching or lifting. Contact with individuals with communicable diseases. Requires the ability to work under stressful conditions. Ability to hear normal range of voice. Ability to prepare written reports and use of telephone and other office equipment such as copier, fax. Requires ability to drive to patient homes and to negotiate steps, walkways and housing not designed for handicap access.

Typical Working Conditions

Normal office environment. Some potential safety risk involved in visiting patients in their home environment. Exposure to communicable diseases and other conditions in a clinic environment. Occasional evening or weekend work.

Job Type: Full-time

Pay: $62,000.00 - $95,750.00 per year
