POSITION OVERVIEW:

Ritter Center is hiring an Enhanced Care Management Project Coordinator for our Whole Person Care Department.

**This role is based in SAN RAFAEL, MARIN COUNTY, NORTHERN CALIFORNIA**

This time delineated One year-long special project slated to start January 1, 2023, thru December 31, 2023, for developing and implementing Cal-AIM Enhanced Care Management (ECM) practices for Ritter Center’s Housing Based Case Management Team (Housing First Model). Primary responsibilities will include: understanding the current Housing Based Case Management Team and processes, learning ECM program requirements and protocols, and transition the program into ECM format.

This is a NON-EXEMPT position that reports to the Program Manager for Whole Person Care.

PRIMARY RESPONSIBILITIES:

  • The ECM Project Coordinator will need to educate themselves on all aspects of the Cal-AIM ECM program to become the Ritter Center ECM expert quickly (understand grant deliverable, reporting requirements, billing guidelines, and funding limits)
  • Learn the Partnership Health Plan (PHP) terminology (TARs, RAFs, RFTs, LOIs)
  • Manage and track TARs, referrals, RFTs, LOIs, and ROIs
  • PHP template expert for charting
  • Develop a policy and procedure manual containing:
  • Support for the range of ECM and case management outreach activities
  • Population engagement protocols
  • Procedures for linking enrollees to medical/behavioral health services
  • Billing activity procedures
  • Documentation/reporting process for new enrollees
  • Patient data management and monthly reporting requirements
  • Necessary safety protocols
  • Implement best practices to provide comprehensive and high-quality support to ECM enrollees
  • Train staff on all ECM protocols i.e. policies, PHP templates, procedures, etc.
  • Provide oversight and support to the Housing Based Case Management team regarding ECM reporting, documentation, and care plans
  • Remain current about the ECM program/updates, quality assurance efforts, trainings, policies/procedures by attending webinars, receiving emails and subscribing to relevant newsletters, etc.

QUALIFICATIONS & REQUIRED KNOWLEDGE:

  • Per OSHA requirements, employment at Ritter Center requires compliance with being able to produce a negative TB test prior to the start of employment, as well as being fully vaccinated against COVID-19 (unless applying for an exemption) per the August 5th State Public Order for Healthcare Facilities, providers, and employees.
  • Equivalent of a four-year degree from an accredited college or university in sociology, psychology, counseling, or a closely-related field and three years of experience in mental health, social services, or a community services organization. Significant experience may be considered a substitute for education. Master’s degrees in a related field may be substituted for work-related experience.
  • Principles and techniques of Housing First and Harm Reduction concepts and the social service and mental health fields.
  • Ability to navigate public assistance, social service programs, and public housing practices to help coordinate care and communicate client needs effectively.
  • Excellent interpersonal communication skills; able to build and maintain long-term effective rapport with individuals.
  • Meticulous record-keeping via online databases. Previous experience in HMIS data base system is preferred.
  • Exceptional organizational skills
