JOB SUMMARY:

The Endocrine Nurse Practitioner will provide specialty level endocrine/diabetes care in the outpatient clinical setting. Utilizing advanced decision making skills and expertise, the Endocrine Nurse Practitioner will formulate, initiate and modify endocrine care plans working in collaboration with designated supervising physician to promote optimal patient outcomes and utilization of available resources. The Endocrine Nurse Practitioner provides clinical resources, education and consultation to staff and demonstrates advanced clinical competence in diabetes and endocrine care.

Marin Health Endocrine & Diabetes Care, a UCSF Health Clinic provides state of the art endocrine consultative care for patients with endocrine and metabolic disorders.

We are a highly respected practice with a 34-year history in Marin County, looking for a full time Nurse practitioner, CDE. with a strong background in diabetology to serve the needs of our very full practice. You will be practicing alongside of our board certified endocrinologists, one of whom co-founded and directs the Braden Diabetes Center and Marin Health Diabetes Care Program. We have a wonderfully strong support staff with years of history within our practice.

The candidate qualifications are:

  • Strong philosophy of excellence in patient care and overall patient experience.
  • Licensed in California
  • (CDE preferred but willing to assist)
  • Spanish language skills a plus but not required.
  • Knowledge of various diabetes technologies such as Dexcom, Medtronic pumps and Libre readers

Patient care includes consultative services, follow up care and support working with our endocrinologists, patient education, prevention and treatment of diabetes, glycemic issues, glandular diseases, hormonal and women’s health and osteoporosis.

JOB SPECIFICATIONS:

EDUCATION

  • Graduate of accredited school of nursing.
  • Completion of an accredited Nurse Practitioner Program with minimum of MSN degree.
  • Board certification by ANCC or ANPP as an advanced practice nurse/nurse practitioner in the specialty areas of Family or Adult NP.
  • Certified Diabetes Educator (CDE) preferred.

EXPERIENCE

  • Preferred 2 years’ previous experience in an advanced practice NP role, with demonstrated expertise in diabetes management and technologies and/or endocrine care.
  • Preferred experience with the evaluation and management of non-diabetes endocrine conditions.
  • Working knowledge of American Diabetes Association (ADA) stands of care.
  • Expert knowledge of key patient care standards and compliance specifications, Title 22, and Joint Commission.
  • Experience in electronic medical records, data management and analysis preferred.

PREREQUISITE SKILLS

  • Demonstrated knowledge of education theory and principles and methods of adult education.
  • Demonstrated knowledge of current ADA recommendations for patient management.
  • Demonstrated teaching and leadership abilities.
  • Demonstrated oral and written skills.
  • Demonstrated computer skills.
  • Demonstrated presentation skills.
  • Demonstrated ability to work collaboratively and develop positive interactions with patients, families, physicians, staff and other disciplines.
  • Ability to read, write, understand and follow English verbal and written instructions.
  • Ability to set priorities, achieve objectives, solve problems, meet deadlines, coordinate simultaneous projects, and make decisions.

LICENSE/REGISTRATION/CERTIFICATION:

  • Current California RN and Nurse Practitioner licenses.
  • Certification by ANCC or ANPP as an advanced practice nurse/nurse practitioner.
  • CDE license (preferred
  • BLS-HCP within 90 days of hire.

DUTIES AND RESPONSIBILITIES:

ESSENTIAL (not modifiable)

  • Evaluates and manages the care of patients and endocrine conditions in the outpatient setting.
  • Obtains complete histories, performs pertinent physical exams, and evaluates diagnostics with assessment of normal and abnormal findings.
  • Administers, orders, and furnishes and/or prescribes medications.
  • Orders and collects specimens for laboratory and imaging evaluation.
  • Formulates and delivers education and support with making lifestyle modifications to patients and their families.
  • Facilitates referrals to interdisciplinary care team. Assist in staff education including new nurse orientation, grand rounds, and CME events.
  • Assists physician colleagues with patient care coordination including patient communication, review of labs, phone triage for urgent patient concerns, and post-visit follow up.
  • Assists in the development of instructional material for use in diabetes and endocrine care.
  • Demonstrates clinical expertise in diabetes and endocrine management by serving as a clinical expert, role model and clinical consultant to patient care staff, physicians and other ancillary staff as appropriate.
