Overview

Under the supervision of the Enhanced Care Management (ECM) Director, the Case Manager will be responsible for providing comprehensive case management and support services to the patients of the organization. The Case Manager will work collaboratively with individual patients to achieve better quality health outcomes.

Responsibilities:

1. Collaborates with primary care providers, patient and patient’s family or support systems, homeless outreach team, and community based hospitals and homeless service providers to provide enhanced care coordination for eligible Health Homes Program members.

2. Conducts case management services to include comprehensive intake and assessment, implementation of care plan, subspecialty referrals, monitoring and follow-up.

3. Conducts initial case management assessment on all newly enrolled HHP members.

4. Develop, implement, monitor and maintain a comprehensive care plan with input from LCSW & PCP’s on an ongoing basis.

5. Maintains and provides appropriate referrals for subspecialty care per case management protocol (i.e. ophthalmology, behavioral health, podiatry, nutrition, health education, etc.)

6. Provide referrals, coordinate services and reduce barriers including transportation, help at home, meals, medications, finance and insurance challenges as well as many others.

7. Collects and enters necessary data for reporting purposes, completes and submits monthly project activity and other reports as assigned for submission to funding source(s) or JWCH CQI Committee.

8. Participates in agency and community meetings pertaining to the needs of patients and issues related to Case Management diseases.

9. Participates in weekly case conferences with PCP’s clinical supervisor and peers to assist in problem solving related to clients and to ensure that professional guidance and high-quality case management services are being provided.

10. Performs other duties as assigned.

Experience Requirements:

1. Master’s Degree in Social Work with at least 1-year experience or Bachelor’s Degree in Social Work or Sociology with 3 years’ experience.

2. Experience as case manager in health setting working with patients who have multiple chronic medical conditions, multiple ER visits or IP visits, homeless or at risk of homelessness and/or adults with serious mental illness or substance use disorders.

3. Electronic Health Record experience and/ Data entry skills required.

4. Proficient in utilizing Microsoft Word, Excel and Power Point. 5. Good communication and customer service skills.

6. Strong desire to work as part of multi-disciplinary team including physicians, LCSW, MSW, RN’s, LVN’s and Medical Assistants.

7. Bilingual (English/Spanish) ability strongly preferred

Job Type: Full-time

Pay: $63,000.00 - $75,000.00 per year
