• Demonstrates knowledge of human growth and development, assessment, types of treatment, and care of patients appropriate to the ages of patients. ..

• Promotes internal and external customer services initiatives

• Utilizes critical thinking skills to meet needs of individual patients.

• Demonstrates ability to perform technical skills and procedures.

• Documents all patient care.

• Participates in Performance Improvement activities as assigned.

• Works with Business Office to manage issue..

• Acts as a liaison with patients, physicians to keep them informed of care issues, such as discharge plans.

• Actively works with interdisciplinary team to assist patient and family with discharge plans.

• RN License (required)

• Demonstrated experience with each of the required areas of expertise in section

• Interpersonal skills to successfully interact with patients, families, hospital staff, physicians, community agency staff

• Case Management Hospital experience
